Support Coordination
Support Connection, Coordination of Supports, and Specialist Support Coordination
Support Coordination is a vital service that empowers participants to navigate and maximize their NDIS plans, facilitating access to a range of supports tailored to individual needs.
Level 1: Support Connection focuses on aiding participants in understanding their plans and connecting with relevant services. Support Connection fosters effective plan utilisation while enhancing participants' capacity to independently engage with support providers and navigate NDIA processes.
Level 2: Coordination of Supports empowers participants to direct their lives by building resilient support networks. This level involves collaborative planning with participants to design personalised support approaches, develop capacity, and promote resilience within their networks.
Level 3: Specialist Support Coordination addresses complex needs and high-level risks. Specialist Support Coordination utilises qualified allied health practitioners to overcome barriers to plan implementation. With a focus on negotiating appropriate solutions and resolving crises, Specialist Support Coordinators ensure well-coordinated support delivery, fostering participant autonomy and well-being.

Here's what support coordination may look like for you at Wired for Wellness:
+ Initial Assessment and Planning: The participant meets with their support coordinator to discuss their goals, preferences, and support needs. Together, they develop a personalised support plan outlining the participant's goals and the types of support required to achieve them
+Connecting with Service Providers: The support coordinator helps the participant connect with relevant service providers and community resources based on their needs and preferences. This may involve researching and contacting service providers, arranging appointments, and coordinating service delivery
+Navigating the NDIS: The support coordinator assists the participant in understanding their NDIS plan, including their funded supports and how to access them. They may help the participant navigate the NDIS portal, submit claims, and communicate with the NDIA as needed
+Building Skills and Independence: Depending on the participant's goals, the support coordinator may provide coaching and guidance to help them develop skills and strategies for managing their supports independently. This may include budgeting, scheduling appointments, and advocating for their needs
+Monitoring Progress: The support coordinator regularly checks in with the participant to monitor their progress towards their goals and adjust their support plan as needed. They may also help the participant identify any barriers or challenges they are facing and work together to address them.
+Advocacy and Support: The support coordinator advocates on behalf of the participant to ensure they receive the supports and services they are entitled to. They may attend meetings with service providers or government agencies, provide information during disputes or conflicts, and help the participant navigate complex systems.
+Empowerment and Encouragement: Throughout the process, the support coordinator provides encouragement, motivation, and emotional support to the participant, helping them stay focused on their goals and overcome any obstacles they encounter.
Wired for Wellness Support coordination involves conducting regular check-ins with both, you and your service providers to ensure the smooth running of services. As per the NDIA guidelines, numerous progress reports reports will be authored at key milestones, and as your plan approaches its conclusion, a meeting is scheduled in order to discuss your future goals and support needs.
At Wired for Wellness, we're here to assist you in navigating the complexities of the ever-evolving NDIA system. Please contact us via the enquiry form on the Contact Us page, or call us on 0490 389 368.
